Short answer: in Maryland, doctor-supervised clear aligners usually cost $3,000 to $8,000 in 2026, with minor touch-up cases starting around $2,500. CareCredit lists the average clear-aligner cost in Maryland at about $5,839, compared with $5,108 nationally (CareCredit) — a gap that mostly reflects higher overhead in the D.C. and Baltimore suburbs.

That headline number hides the decisions that actually move your price: which kind of aligner you get, who supervises it, and what the fee includes. This guide breaks those down for Maryland patients. If you already know you want Invisalign specifically, our Invisalign cost in Maryland guide has practice-by-practice prices by county.

Invisalign

The best-known brand and the one most Maryland orthodontists and many general dentists offer. Published local ranges include $3,000–$7,000 in Bethesda, $3,500–$7,000 in Silver Spring, and $3,500–$8,000 in Baltimore and Frederick (details and sources in our Invisalign cost guide). Providers carry experience tiers based on case volume, which is a useful signal of how many cases an office has finished.

Other doctor-prescribed brands

Spark, ClearCorrect, Angel Aligner, and similar systems work the same way: a licensed provider scans your teeth, plans the movement, and monitors you in person. Some Maryland practices price these a few hundred dollars below Invisalign; many charge the same flat fee regardless of brand, because the doctor’s time — not the plastic — is most of the cost. Ask which systems an office uses and why they would choose one for your case.

In-office 3D-printed aligners

A growing number of practices design and print aligners in-house. For simple cases this can cut the fee meaningfully, and replacement trays are fast. The trade-off is that these are best suited to mild movements; complex bite correction usually still calls for a full-scope system.

Mail-order aligners: the cheapest sticker, the highest risk

Remote aligner companies advertised prices well under $2,500 by skipping the in-person exam. That model took a hit when SmileDirectClub filed for bankruptcy in 2023 and shut down that December, leaving patients mid-treatment without support or refunds. Other direct-to-consumer brands have since shifted toward dentist-involved models or scaled back.

The underlying problem is medical, not just financial: moving teeth without checking gum health, bone levels, and the bite can cause problems that cost far more to fix than you saved. The American Association of Orthodontists recommends an in-person exam before any orthodontic treatment (AAO). In Maryland, dentistry — including orthodontic treatment — is regulated by the Maryland State Board of Dental Examiners, and you can look up any provider’s license there before you pay.

Dentist or orthodontist: does it change the price?

Less than people expect. In Montgomery County, general dentists and orthodontists often quote similar fees for comparable aligner cases. What differs is training: orthodontists complete two to three additional years of residency focused solely on moving teeth and correcting bites. A practical rule:

  • Mild crowding, spacing, or relapse — an experienced general dentist with a high aligner case volume can be good value.
  • Bite problems, significant crowding, rotations, or a growing child — see a specialist orthodontist. A cheaper quote that cannot finish the case is not cheaper.

The hidden costs that make quotes hard to compare

Two Maryland quotes of $4,200 and $5,900 may be for very different packages. Get every line in writing:

Line itemIncluded?Typical cost if extra
Consultation, X-rays, and digital scanAsk$0–$350
All aligners in the initial seriesUsually—
Refinement aligners (mid-course corrections)Ask$300–$1,500
Attachments and enamel reshapingUsually—
Progress check-upsAsk$50–$150/visit
Lost or broken tray replacementAsk$50–$150/tray
Retainers (first set)Ask$300–$1,000+
Treatment extension if you finish lateAskVaries

The two lines that most often separate a “cheap” quote from a fair one are unlimited vs. capped refinements and whether retainers are included. Retainers are a lifetime commitment, so ask what replacements cost too.

How Maryland patients lower the out-of-pocket price

Dental insurance. Orthodontic coverage, where it exists, is usually a lifetime maximum of about $1,000–$2,000 paid at roughly 50%, often with age limits for adults (Delta Dental). Most plans treat aligners the same as braces. Ask the office to submit a pre-treatment estimate before you sign. Our guide to affordable dental care and insurance in Maryland covers how to read a plan.

FSA and HSA dollars. Orthodontics to correct alignment is a qualified medical expense (IRS Publication 502). For 2026 the health FSA limit is $3,400, and HSA limits are $4,400 (self-only) and $8,750 (family). A Maryland household that times open enrollment can cover most of a typical case pre-tax — and many practices will split the fee across two plan years.

Maryland Medicaid. Maryland Healthy Smiles can cover medically necessary orthodontics for eligible members under 21, with prior authorization based on severity scoring. Cosmetic adult aligners are generally not covered, and aligners are not automatically approved even when braces would be.

Payment plans. Most Montgomery County practices offer in-house, often interest-free, monthly plans; local advertised payments commonly start around $130–$185/month. Compare the total contract, not the monthly figure, and be cautious with deferred-interest medical cards — interest can be charged retroactively if the balance is not paid off in time.

Two-quote rule. Pricing varies widely even between neighboring towns. Getting a second consultation — often free — is the single easiest way to save, and it also tells you whether the first diagnosis holds up.

Clear aligners vs. braces on price

In Maryland, aligners and braces are usually within a few hundred dollars of each other for the same case; metal braces are often the lowest-cost full-scope option, and lingual braces the highest. See the full numbers in our braces cost in Maryland 2026 guide, and the trade-offs beyond price in Invisalign vs. braces in Bethesda and Rockville. Aligners only work if worn 20–22 hours a day; if that is unrealistic, braces are often the better value.

Questions to ask at your consultation

  1. Am I a good aligner candidate, or would braces finish my case better?
  2. What is the total fee, and exactly what does it include?
  3. How many refinements are included, and what happens if I need more?
  4. Are retainers included, and what do replacements cost?
  5. Which aligner system will you use, and how many aligner cases have you completed?
  6. Will you submit a pre-treatment estimate to my insurance?
  7. Can the fee be split across two FSA/HSA plan years?
